Our Union, Our Future - Dues Survey

**This is not an official vote and will not change the current dues rate. It is a survey to understand member preferences.**

Survey opens November 5 at 9am and closes November 16 at 8pm

Survey is open to UFCW 663 members who work in the Metro and Brainerd area

Login Instructions

1. Your access key is the last 6 digits of your social security number and the first three letters of your last name. The capitalization of the 3 letters does not matter. Make sure there are no empty spaces.
2. For example, if the last 6 of your SSN is 123456 and your last name is Smith, your access key would be 123456SMI.
3. Your password is UFCW
